APPLICANT REQUESTS THAT:
1. His records be corrected to reflect his middle initial as O
rather than D.
2. His records be corrected to reflect award of the Vietnam
Service Medal (VSM) and any other awards he may be entitled to
(administratively resolved).
APPLICANT CONTENDS THAT:
His middle initial is O not D.

STATEMENT OF FACTS:
The applicant served on active duty in the Regular Air Force from
24 May 72 through 24 May 76.
On 14 Jan 14, AFPC/DPSIRP notified the applicant that his DD Form
214 reflects his middle name as Oliver; therefore, there was no
basis to correct his DD Form 214.
On 19 Feb 14, AFPC/DPAPP notified the applicant that his foreign
service boots on ground in Thailand had been verified; however,
they were unable to verify any foreign service for Vietnam.
AFPC/DPSID has advised that applicants records will be
administratively corrected to reflect the Air Medal, Air Force
Outstanding Unit Award with Valor and one Bronze Oak Leaf Cluster,
Vietnam Service Medal with one Bronze Service Star, Air Force
Longevity Service Award, and the Republic of Vietnam Gallantry
Cross with Palm.
The remaining relevant facts pertaining to this application are
contained in the memorandum prepared by the Air Force office of
primary responsibility (OPR), which is attached at Exhibit C.
AIR FORCE EVALUATION:
AFPC/DPAPP recommends denial for foreign service in Vietnam. A
review of the applicants master personnel records did not
substantiate he served in Vietnam. However, boots on ground was
verified and confirmed for Thailand.
A complete copy of the AFPC/DPAPP evaluation is at Exhibit C.
APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant
on 9 Jun 14 for review and comment within 30 days (Exhibit D). As
of this date, no response has been received by this office.
THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice regarding the
portion of the applicants request to correct his DD Form 214 to
reflect his middle initial as O. We note a review of the
applicants personnel records revealed his middle name on his DD
Form 214 was Oliver; therefore, in view of the fact the
applicants middle name as reflected in his records is Oliver, and
there is no entry that we can find in the records that indicates
his middle initial is D, we find no basis to recommend the
requested relief be granted. As for his request for awards
related to his Vietnam service, we note the Air Force office of
primary responsibility has determined the applicants entitlement
to Air Medal, Air Force Outstanding Unit Award with Valor and one
Bronze Oak Leaf Cluster, Vietnam Service Medal with one Bronze
Service Star, Air Force Longevity Service Award, and the Republic
of Vietnam Gallantry Cross with Palm and will correct his records
administratively. Therefore, we find no basis to recommend
granting relief beyond that rendered administratively.
THE BOARD DETERMINES THAT:
The applicant be notified the evidence presented did not
demonstrate the existence of material error or injustice; the
application was denied without a personal appearance; and the
application will only be reconsidered upon the submission of newly
discovered relevant evidence not considered with this application.
